If you know someone who could benefit from DiverseAccessCare's services, we make the referral process straightforward. Complete the form below with the participant's details and we will handle the rest. All information is treated with the strictest confidence in accordance with the Australian Privacy Act 1988.

What Happens Next
Once we receive your referral, our team follows a clear, structured process to ensure the participant receives the right support as quickly as possible.
Referral Acknowledgement
We acknowledge receipt of your referral within one business day. You will receive a confirmation email with a reference number and the name of the team member assigned to the case.
Initial Assessment
Our intake team reviews the referral and contacts the participant (or their representative) to conduct an initial needs assessment. We discuss goals, preferences, current supports, and any specific requirements.
Service Agreement
We prepare a tailored service agreement outlining the supports to be delivered, scheduling arrangements, pricing in line with the NDIS Price Guide, and cancellation terms. The agreement is reviewed and signed by the participant or their nominee.
Support Worker Matching
We carefully match the participant with a support worker based on their needs, personality, cultural preferences, and location. Where possible, we introduce the support worker before services commence so the participant feels comfortable.
Services Commence
Support begins according to the agreed schedule. We conduct regular check-ins to ensure the participant is satisfied with the service quality, and we remain in communication with the referring party to provide progress updates as appropriate.
